Community Service Requirement Waiver
This form is to be used by individuals who believe that they meet the requirements to be exempt from the Community Service Requirement. This form is for a single individual. If more than one household member believes they are exempt, you may make copies of this form so that each individual can submit their own form. Or, you may call our offices at (301) 929-5667 for additional copies of the form.
Community Service Quarterly Report
This form is used to document the number of Community Service hours each individual performs. Each household member who is required to perform Community Service must submit his or her own form. A sample form has been included, as well. A new form will be sent to you once you mail in a completed form. For additional forms, call the Community Service Information Line at (301) 929-5667.
