Application Process

Prospective participants are required to submit an application to HOC’s Housing Path waitlist. Please review the current income and rent limit amounts that are used to determine income eligibility requirements for renting a Scattered Site home.

Application with Guidelines Income & Rent Limits

Please note: you must provide the following items to qualify:
  • Picture ID for all adult members 18 years of age & older
  • Birth Certificates for all members
  • Social Security cards for all members
  • Proof of Income from all sources for the last two months
  • Checking: Last six months of bank statements — all pages
  • Savings: Most recent bank statement — all pages
  • Proof of all assets
    • 401k
    • IRA
    • CD
    • Trust accounts
  • Voucher Packet — if applicable
